Thank you for joining us as we all work together to conquer breast and cervical cancer. We have designed this site to be a “communications hub” for the exchange of information and ideas. With funding from the Centers for Disease Control, the Partnerships will focus o cancer screening education, evidence-based systems change in primary care and cancer care clinical settings, and community-wide connections to encourage seamless patient navigation from screening to diagnosis to treatment and to survivorship programs. We’ll spotlight federally-qualified health centers and Nebraska public health departments that are forming strong partnerships with local hospitals, accredited cancer centers, and evidence-based survivorship programs to create a collaborative continuum of care to eliminate disparities.
We have included measurable goals and objectives from the 2017-2022 state plan and will keep you posted on activities initiated by work groups formed around them.
